plan, as well as optional stand-alone dental and vision plans. Following

Notes:
Medical options depend solely on individual employee needs. All
full-time employees qualify for Life & Disability coverage through
our company and may also take advantage of our 401(k) retirement
savings program.
All benefits are available to employees the 1st of the month following
90 days full-time, continuous employment with PML.
Any client with an existing Blue Cross/Blue Shield group
health plan may transfer to one of PML’s plans effective the 1st
of the month following enrollment with one of our affiliate group’s
payrolls*. Other transfers of coverage are dependent on the carriers
rules. All other benefits must meet carrier requirements and open
enrollment requirements. The 401(k) eligibility requirement
is one year on payroll, with an average of 1,000 hours per year
and enrollee must be 21 or older to participate. (*Note: The HMO
Plans must be Michigan plans to transfer to PML's HMO plans).
