PML Holdings Group, LLC, offers three different Blue Cross/Blue Shield health benefits options, combined with a stand-alone prescription plan, as well as optional stand-alone dental and vision plans. Following is a short overview of coverage offered under these plans.

Health Care Plans



Blue Cross/Blue Shield PPO 2*   (View Details)
• $15 office visit co-pay, Preventative Services covered 100% up to $250/yr
• 90% hospitalization after deductible
• $100 deductible per member/$200 per family per year
• Out of Network Benefits are reduced

Blue Cross/Blue Shield PPO 4*   (View Details)
• $15 office visit co-pay, Preventative Services covered 100% up to $500/yr
• 80% hospitalization after deductible
• $500 deductible per member/$1000.00 per family per year
• Out of Network Benefits are reduced



Blue Care Network HMO* (Available in Michigan Only)  (View Details)
• $15 office visit co-pay, Preventative Services covered 100% after co-pay
• 25% hospitalization co-pay w/max $1,000 per member/$2,000 per family out-of-pocket
• There is no annual deductible
• No out of network benefits

* All Blue Cross PPO and Blue Care Network plans require a stand-alone prescription plan.



HAP - Health Alliance Plan HMO (Available in Michigan Only)
• 100% Office Visit
• 100% Unlimited Inpatient Hospital Services
• Built-in Prescription Plan with $5 Co-pay
• HMO - POS (Point of Service) Option

NOTE: Blue Care Network & HAP HMO's are only available in Michigan

Prescription Plan

E.H.I.M. Employee Health Insurance Management
• Pharmacy - 30 day supply: $15 Brand Name co-pay, $5 Generic co-pay
• Pharmacy - 90 day supply: $15 Brand Name co-pay, $5 Generic co-pay

Dental Plan



MMA/MetLife Preferred Provider Dental
• 100% Preventative: Exams, Cleaning & Scaling
• 80% Basic: X-rays, fillings, extractions, periodontal treatment (after ded.)
• 50% Major & Orthodontia: Crowns, dentures, bridgework (after ded.)
• $50 deductible per member/$150 per family per year

Vision Plan



VSP - Vision Service Provider
• 100% annual eye exam after $10 co-pay
• Corrective lenses every 24 months after $25 materials co-pay
• Frames every 24 months (up to plan allowance)
• Out of Network benefits are reduced

401 (k) Plan

Information on our 401(k) plan is currently unavailable - New regulations coming in 2004

Notes:

Medical options depend solely on individual employee needs. All full-time employees qualify for Life & Disability coverage through our company and may also take advantage of our 401(k) retirement savings program.

All benefits are available to employees the 1st of the month following 90 days full-time, continuous employment with PML.

Any client with an existing Blue Cross/Blue Shield group health plan may transfer to one of PML’s plans effective the 1st of the month following enrollment with one of our affiliate group’s payrolls*. Other transfers of coverage are dependent on the carriers rules. All other benefits must meet carrier requirements and open enrollment requirements.  The 401(k) eligibility requirement is one year on payroll, with an average of 1,000 hours per year and enrollee must be 21 or older to participate. (*Note: The HMO Plans must be Michigan plans to transfer to PML's HMO plans).
